"NSAIDs are extremely effective for controlling arthritis pain and inflammation in dogs," says Stephen F. Sundlof, D.V.M., Ph.D., director of the Food and Drug Administration's Center for Veterinary Medicine (CVM). "These are very valuable arthritis drugs that help many pets live to a ripe old age."
But like any drugs, NSAIDs carry a risk of side effects, or adverse reactions. Most adverse reactions are mild, but some may be serious, especially if the drugs are not used according to labeled directions. Some reactions result in permanent damage or even death.
"It's important for pet owners to be aware of the risks and benefits of all arthritis drugs, including NSAIDs, so that they can make informed decisions about their pets' health care," says Sundlof. "Owners who give their dog NSAIDs need to know the side effects to watch for that indicate their pet needs medical attention."
"The most common side effects from NSAIDs (nonsteroidal anti-inflammatory drugs) include vomiting, loss of appetite, depression, lethargy, and diarrhoea. Serious side effects include gastrointestinal bleeding, ulcers, perforations, kidney damage, and liver problems."
"The side effects of NSAIDs are very well known and very well documented," says Michele Sharkey, D.V.M., in the CVM's Office of New Animal Drug Evaluation. But this information is not always getting to the pet owner, she says. "If the pet owner can recognize a possible reaction, stop the arthritis medication, and get veterinary help, it could mean the difference between a good outcome and a disaster."
There is some evidence that not all vets are aware of the potential problems with these arthritis drugs and are not able to recognize the symptoms of adverse reactions and side effects.
Treatments such as arthritis oils and creams, applied externally, which are sometimes effective with humans are much more difficult to apply to dogs, especially if the coat is thick. Various other alternatives to anti-inflammatory drugs are on the market, including drinks and "arthritis natural" ingredients.
Glucosamine is a very popular ingredient, sometimes included with chondroitin. I know many aged golfers who swear by it!
A company called Maxavita claims that its arthritis product, called Pernamax, which comes in the form of caplets or gel, is 200 times stronger than glucosamine. It includes the powdered extract of the New Zealand Green Lipped mussel.
None of these arthritis remedies are cheap and all involve a continuing cost, which over a year can be very substantial, especially if there is a need for the vet's involvement.

Dogs and Arthritis: The Causes
This potentially very painful condition is common in older dogs because over time, wear and tear affects the joints. Younger dogs can also develop arthritis, although less frequently. Unlike the arthritis in older dogs that is often caused by ordinary wear and tear, accidents, injuries and certain infections, diseases and other health conditions like joint malformations (hip dysplasia, for example) are usually responsible for arthritis occurring in younger dogs. Arthritis can also be insidious in the sense that it might not become apparent for several years following an injury.
Although there are several forms of arthritis, the underlying cause of each type is inflammation in a joint. The different types of arthritis are categorized by what originally caused this inflammation.
Degenerative joint disease, frequently called osteoarthritis, is far and away the most common form of arthritis in dogs. Frequently associated with aging (as it is in people), osteoarthritis typically develops over the course of time. Ordinary wear and tear or injury to a joint can damage the joint's ligaments and/or cartilage and reduce the production of the synovial fluid that lubricates and protects the bones. The ultimate result is joint tissue degeneration, stiffness and pain. Cartilage damage and subsequent arthritis can also be caused by excessive weight, jumping too frequently over obstacles, tearing or stretching ligaments during vigorous exercise, and joint malformations such as hip dysplasia or "loose" kneecaps, among other things.
The mechanism underlying the second main type of arthritis, called inflammatory joint disease, is different than the mechanical stress or trauma that's responsible for degenerative joint disease. It is also much less common than osteoarthritis. This form of canine arthritis can be caused by a bacterial infection (staphylococcus or streptococcus, for example), a tick-borne disease such as Rocky Mountain spotted fever or a fungal infection in a joint, but it can also be caused by auto-immune disorders, some of which are hereditary. Rheumatoid arthritis is one example of arthritis caused by an auto-immune disorder, but fortunately it is rare in dogs. Arthritis resulting from an auto-immune disorder usually affects multiple joints.
Inflammatory joint disease is often characterized by systemic symptoms such as fever, loss of appetite and a generalized, overall stiffness. Unlike the form of arthritis that degenerative joint disease typically causes, which usually has a slow, gradual onset, the appearance of arthritis symptoms due to inflammatory joint disease is usually sudden and acute.
Symptoms of Arthritis in Dogs
The symptoms of arthritis in dogs are in general analogous to those of arthritis in humans, if you take into account the differences in anatomy. They can vary in severity from being barely detectable to being crippling and debilitating. Obviously, dogs cannot tell you they're hurting, and most do not show any visible signs of pain until it becomes significant. These factors can make it difficult to accurately diagnose arthritis unless X-rays, an orthopedic exam and other more specific tests are performed by a qualified vet. You should never assume that your dog has arthritis just because it comes up lame one day or gets up slowly or stiffly after sleeping all night. Although it's true that those symptoms could mean your dog has arthritis, they also might be signs of an entirely different health condition. Always take your dog to your veterinarian for assessment, diagnosis and treatment.
Notwithstanding the difficulty of making an accurate diagnosis, dogs suffering from arthritis typically exhibit one or more of the following symptoms of arthritis in dogs:
Slower gait: Dogs afflicted with arthritis often walk slowly in an attempt to balance out their limbs, especially when more than one joint is affected.
Dropped hip or nodding head: When only one hip is affected, the dog will often "drop" that hip so that it is carried physically lower than the other. A dog with an arthritic neck will often exhibit a nodding motion of the head.
Limping: A dog with an hip arthritic joint in a leg will naturally tend to favor that leg.
Reluctance to engage in normal activities: Dogs with arthritis tend to be reluctant to perform what were previously normal activities, such as climbing up or down stairs, running, jumping up on furniture, playing with balls or other toys, and going on walks. When they do go on walks, they often lag behind because they prefer to walk slowly.
Slowness or difficulty in rising from resting positions: Getting up slowly or stiffly after lying down is one of the fairly common symptoms of arthritis in dogs.
Personality changes: Some dogs with arthritis undergo changes in their personality. Becoming lethargic, depressed, nervous or overly aggressive are examples of the changes in temperament that can occur in dogs afflicted by arthritis. And, a previously very affectionate dog that loved physical attention from its owner may start resisting being touched.
Audible signals of distress: A dog that's in significant pain may yelp, whimper or cry when it's touched or it moves in a certain way.

Although arthritis is not a health condition that can ever be completely cured, there are a number of things you can do to help your dog suffer less from painful arthritis symptoms. Some well known NSAIDs are, Arthrotec (Diclofenac/Misoprostol), Ansaid (Flurbiprofen) and Cataflan (Diclofenac potassium). These three arthritis drugs are used to treat the pain and inflammation of arthritis. Arthrotec combines diclofenac with misoprostol. The misoprostol is added for the purpose of lowering the possibilty of stomach problems.
The NSAIDs work by reducing inflammation in the arthritis affected joints. They do this by blocking an enzyme, which is highly involved in the inflammation process, it is called cyclooxygenase. This converts arachidonic acid to prostaglandins and leukotrienes. Prostaglandins cause some of the inflammations. So, by stopping the cyclooxygenase, the NSAIDs reduce any inflammation.
Any one type of arthritis drug might react differently to a particular person. If you are prescribed arthritis drug, and it doesn't work for you, there is probably another form of it that might react in a better manner to your body.
Some years ago one of the COX-2 selective inhibitor drugs, Vioxx, was voluntarily withdrawn from the market. It was proven that long-term use could increase risk of heart attack and stroke. After this, a review by the FDA deemed that all non-aspirin NSAIDs must carry a warning, and It is important to discuss the potential risks with your doctor. There is now only one COX-2 inhibitor on the market, Celebrex.
About 60% of patients will react to NSAID treatments, and usually a trial period of about three to four weeks should allow enough time to know if the drug is working.
It is possible that NSAIDs can hide the signs and symptoms of other infections.
Some of the adverse effects of NSAIDs are kidney failure, liver malfunction, bleeding, and stomach ulcers.
NSAIDs can interfere with drugs used to control high blood pressure and cardiac failure. If you are taking medication for a heart complaint, or high blood pressure, then it is essential that you tell your doctor, so that he/she will prescribe the correct drug for you.
Some other drugs, such as warfarin, react with the NSAIDs, and so, again, you should discuss any new medications with your doctor before starting them, and visa versa you should always tell any other doctor treating you that you are taking NSAIDs.
You should not take NSAIDs if you are allergic to aspirin.
It is not possible to predict which NSAID will be best for you. Non of them have been proven to be better than any other for pain relief. Once an NSAID has been selected, the dosage can be increased gradually until pain is relieved or until the maximum advised dose has been reached. You should use the lowest dose possible, that relieves your suffering.

I have got to do something about my insomnia. I wake up every night at about 3 am and can’t get back to sleep. I lay there and worry about work and life, and then I worry about my insomnia and the fact that I am going to be exhausted the next day. It’s a cycle that I can’t get out of! I try warm milk, warm baths, and lavender aromatherapy even, but nothing is working. I keep finding myself laying on the couch at 3 in the morning watching Boulder DIRECTV and trying to get sleepy enough to go back to sleep. My doctor prescribed something for me a few weeks ago, but I took it only once because it knocked me out and I could barely function at work the next day. The medicine seemed to actually be worse than just being tired the next day. I’ve got to figure out something, though. Maybe I need to go to counseling to discuss my anxiety. Maybe that is it.
